When both partners need support, care gets more complex. This package extends the full Concierge experience to couples, with dedicated case management that considers the needs of both the person living with dementia and their partner — because caregiving takes a toll on everyone in the home.

Our most intensive social work package — designed for the most complex situations. Whether you're navigating challenging behaviors, managing multiple care partners, or supporting a more independent elder who still needs close oversight, this package provides weekly presence, comprehensive case management and coordination.

Our most comprehensive level of care — and the only package that combines the clinical expertise of a Registered Nurse through Joy Nursing with the depth of a Licensed Clinical Social Worker, both Certified Case Managers. Built for individuals with complex medical and psychological needs, this package is especially valuable during high-stakes transitions: hospital to rehab, rehab to home, or any period requiring close clinical oversight.
Twice-weekly home visits (one by SW, one by RN)
Weekly medication management and pillbox filling
Coordination and oversight of all home care services
Emotional support and counseling for clients and family members
Goals of care conversations, advanced directive support, and future planning
Behavioral support, coping strategies, and caregiver stress management

Your nurse and social work case managers continue alternating weekly visits, remain available by phone and email during business hours, and keep a steady, experienced eye on everything — so you don't have to do it alone.
