Our most comprehensive level of care — and the only package that combines the clinical expertise of a Registered Nurse through Joy Nursing with the depth of a Licensed Clinical Social Worker, both Certified Case Managers. Built for individuals with complex medical and psychological needs, this package is especially valuable during high-stakes transitions: hospital to rehab, rehab to home, or any period requiring close clinical oversight.

• Twice-weekly home visits (one by SW, one by RN)

• Weekly medication management and pillbox filling

• Coordination and oversight of all home care services

• Emotional support and counseling for clients and family members

• Goals of care conversations, advanced directive support, and future planning
